Shaped Forms

I like my program's looks to be modern. The latest craze is to have pop-up forms with rounded corners, shadow and a gradient header.

I looked for transparency on form or use a PNG file, but it needs to be a menu with choices.
Any suggestions?

Patrice Terrier has produced some of the best looking custom forms and controls you will find.
The products he sells might be a little too pricey for most hobbyists.
However he has graciously provided some tutorials on rolling your own if you're willing to spend some time making your head hurt with learning some SDK programming  Check out the Skinbox example.
