Tracking Version Changes, Bugs, and Feature Requests

Can anyone recommend the software they use for tracking version changes, bugs, and feature requests?  Is there something pre-existing you like or just something built from scratch?

I use Mantis because I can enter reports via a web interface from anywhere.
